Understanding patient unmet needs and therapeutic tolerability for rUTI treatment with bacteriophage therapeutics
Thank you for your interest in this study. The purpose of the study is to better understand the patient’s experience of recurrent, persistent, and chronic urinary tract infection (UTI), willingness to try innovative therapies for treatment and prevention of UTI, and tolerability of different dosing regimes. Because this is a symptoms-based study, the questions asked in this survey are not suitable for those who are consistently symptom-free, even if this is due to long-term treatment.
For the purpose of this study, the term 'recurrent UTI' also captures 'persistent' and 'chronic UTI'.
To see if you might qualify for this study, we need to ask you some questions about your health history and present condition. Please be aware that answering yes to some of these questions does not necessarily exclude you from participation, but the information we collect is important for the study itself. Some of these questions may be sensitive, such as questions about hormonal therapy, and your experience with urinary symptoms.
If you do not qualify for this study, the information you provide will be stored securely until data collection for the study is finished and will then be deleted. If you do qualify for this study, you will be provided with a participant information sheet and an opportunity to consent to participating. If you provide consent, participation will involve completing one additional survey.
Please complete this screening questionnaire once only as the system will flag duplicate submissions. If you do not complete this questionnaire the first time you may return to do it again.
